EXPERIENCE

Charlotte is an experienced senior social worker who can provide case management in different sectors. Charlotte has worked in Special Educational Needs Schools, Hospital Discharge Team for adults, 18-25 Disability Team, Access Team, and the Emergency Duty Team (children and adults). In these different roles Charlotte has worked with children and adults of all ages, with different impairments such as physical disability, cognitive impairments, mental health difficulties and those experiencing substance misuse issues.

Charlotte is passionate about supporting people to reach their goals and to achieve their ambitions and to meet their wellbeing needs. For some it is essential to have additional support and assistance to achieve these outcomes. Charlotte has a good understanding of up-to-date legislation in relation to social care and Human Rights.

Charlotte is experienced in completing Care Act assessments in a holistic person-centred manner, utilising the individuals’ strengths and systems around them. Looking at what each person wants to achieve to meet their wellbeing needs. Charlotte is used to using different communication techniques to support individuals to engage in their assessment, this includes images, communication devices and basic Makaton.

Charlotte line manages social workers, social care practitioners and social work apprentices - supporting them with completing Care Act Assessments.

Charlotte is proficient with managing safeguarding referrals and implementing safety plans, identifying protective factors to reduce the risks whilst long term plans are put in place.

Charlotte has chaired Section 42 meetings and is confident in chairing meetings with family and professionals.

Charlotte has completed Mental Capacity Assessments in creative holistic ways with adults of all ages from the age of 16 years, ensuring the adult remains at the centre of all decision making. She has chaired Best Interest Meetings involving family, friends, and health professionals.
