EXPERIENCE

Following qualification in 2003 Fazelet worked for Central Manchester University Hospital completing a variety of rotations.  These were in acute medicine, general, vascular, prosthetic and neuro rehabilitation. Fazelet has experience of clients with orthopaedic and MSK conditions resulting from trauma. Fazelet has an excellent understanding of complex assessment, treatment and discharge planning. She progressed to a senior position in 2005 enhancing her clinical skills in stroke, neuro and pulmonary rehabilitation.  She led client groups including an upper limb group and cognitive rehabilitation group.

In 2007 Fazelet joined the Stockport Foundation Trust as a Specialist Occupational Therapist working in the neuro inpatient and neuro community team for ages 16 plus. She assessed and devised treatment plans to increase functional independence of clients.   She communicated effectively with professionals recommending equipment, housing adaptations, wheelchairs and specialist seating for those with mobility difficulties.  She has worked closely with social workers, detailing care packages and training carers for example on moving and handling techniques. Fazelet has considerable experience working with wider agencies in the community to provide support and funding to ensure immediate and future needs of clients were met.

Over her career she has supervised staff, provided regular clinical supervision and appraisals along with developing competencies. Fazelet has led training sessions for trainee and rehabilitation assistants, occupational therapists and students. Fazelet has led and organised in-service training and journal clubs, reviewing evidence-based practice. She has also been involved in service delivery and improvement.

Fazelet led the FACETS (Fatigue, Applying Cognitive behavioural and Energy Effectiveness Techniques to Lifestyle) for clients.  She has worked closely with employers, statutory and voluntary agencies on long term goals on improving life skills, return to work and education. She continues to specialise in this area and has embarked on the vocational rehabilitation training with the Greater Manchester neuro – rehabilitation operational delivery network.

During her time working with adults, Fazelet has had experience of a range of acute and chronic conditions. She has completed courses on upper limb rehabilitation, introduction to Bobath, evaluation and treatment of visual perceptual dysfunction in adult brain injury, fitness to drive following a brain injury and splinting. Fazelet has also completed a masterclass on the mental capacity act.  She has had to deal with situations involving safeguarding vulnerable children and adults with involvement in best interest meetings.

As an experienced independent case manager, Fazelet has experience of working with adolescents and adults who have sustained major traumatic head injuries, orthopaedic and nerve injuries.  She manages multidisciplinary teams, team leaders, care packages, and works collaboratively with statutory and independent providers and maintains a strong client focus to obtain the optimal outcome for the client and family.

PERSONAL PROFILE

Fazelet is a mother of three young boys and enjoys spending time with family, travelling and socialising with friends. She continues to work as an occupational therapist working with people to enable them to reach their full potential and improve their quality of life.
