EXPERIENCE

Jenni is a passionate Occupational Therapist with 16 years of experience who has worked across both physical and mental health in acute hospital, rehabilitation, and community-based teams. She has been able to transfer this experience into her role as a case manager and she now works with clients with a wide range of chronic and acute conditions, brain injuries, orthopaedics, and palliative care.

Most recently Jenni has worked in Continuing Healthcare as an Assessor and Complex Case Management for a mixed caseload of adults with physical and mental health conditions and learning disabilities

Most of Jenni's experience has been within the community settings, assessing individuals within their home environments. Jenni is holistic in her approach and always ensures what is important to the client is at the forefront of how she works.

Jenni has had training in coaching skills which has assisted her in supporting individuals with goal setting and motivation. Jenni has also worked in a Family Liaison role supporting newly bereaved families. She builds rapport with ease and works with a kind and compassionate approach.

Jenni has experience in the following areas:

  • Completing home based assessments and prescribing equipment and graded therapeutic interventions.
  • Working as part of a Palliative Care Team.
  • Completing rapid response and admission avoidance assessments.
  • Working with the Dementia Outreach Team assessing and providing care plans for care home residents with complex and challenging behaviours.
  • Working within Older People’s Mental Health Team; community, inpatient and day hospital based.
  • Working within a Forensic inpatient setting.
  • Working in Psychiatric Intensive Care and Low Secure Unit.

In addition to her Occupational Therapy experience, Jenni worked for two years as a Family Liaison Officer for a Mental HealthTrust. This involved supporting families who had been bereaved whilst their loved ones were under the care of the Trust; providing emotional and practical support during the investigation process and Coroner’s Court proceedings.

PERSONAL PROFILE

Jenni started life in Maidstone, Kent and moved to Norwich, Norfolk at a young age. She later moved to Derby to go to university and made Derby her home for the following 15 years. She then returned to Norwich where she married and had two children. Jenni loves living in such a beautiful city with countryside to explore and the coast nearby. Jenni is also a keen cyclist and spends much of her spare time taking her children to their chosen extra-curriculum activities.
