EXPERIENCE

Prior to undertaking case management Louise had nearly 23 years clinical experience as a senior physiotherapist within both the NHS and private sector.

She now case manages adult clients who may have suffered:

  • Complex Neurological Conditions
  • Spinal Cord Compression (Adult)
  • Traumatic Brain Injury (Adult)
  • Parkinsons Disease
  • Multiple Sclerosis
  • Stroke
  • Amputees
  • Orthopaedics
  • Falls and Frailty
  • Cancer and End of Life Care

Louise has experience of carrying out complex holistic client needs assessments across multiple specialties. She has worked with adults of all ages, with complex medical and social needs. She is able to set client centred rehabilitation goals and management plans to maximise client outcomes and promote independence where possible and confidently refer to other specialists where necessary.

Louise enjoys seeing her catastrophic complex injured clients progressing through their rehabilitation journey and onwards to reaching their potential for independent functional ability on a biopsychosocial basis.

This includes environmental visits to establish actual and potential risks, reviewing any incidents and developing staff competences within the community setting, which includes assessing the home for the suitability of prescribed equipment and functional assessments within the home.

Louise has experience of undertaking complex capacity assessments as required, to assist with planning and reviewing care packages. When needed Louise acts as an advocate in the best interests of her clients.

She has a wealth of management experience and regularly chairs and supervises clinical and managerial teams across many services and was a founding member of the award-winning frailty team in South Manchester 2014.

Louise has 3 year’s experience as a senior Physiotherapist in a hospice setting.  Working as part of multidisciplinary team to facilitate the best holistic goals and care for patients approaching the end of their life while supporting their families as well.

She successfully completed the comprehensive Community Case Management Services brain, spinal and catastrophic complex injury case management pathway training, which includes a comprehensive understanding of the litigation process.

Louise is a lateral thinker and practical problem solver who is passionate about maximising individual potential.

PERSONAL PROFILE

Louise has three children and as the mother of a child with special educational and social needs she understands first-hand the difficulties challenging behaviour can bring to families.  She enjoys learning complimentary therapies and making natural cosmetics in her spare time.  She likes baking and exercise classes when family commitments allow.
