EXPERIENCE

Simone studied Psychology at Nottingham TrentUniversity, and then following her undergraduate degree she completed a master’s degree in Occupational Therapy. She currently works part-time at Queen Elizabeth Hospital across Oncology and Haematology, healthcare for older people, trauma, acute medicine, specialist surgery, stroke and neurosciences. Simone demonstrates excellent knowledge and skills in complex assessment, treatment/intervention and safe and effective discharge planning within acute medical settings. Simone empowers people to facilitate recovery and overcome barriers preventing them from engaging in the activities and occupations that provided meaning and purpose to them.

Simone has over 8 year’s experience working as a rehabilitation assistant and team leader. Simone provided specialised support for young children and adults living in the community with a range of complex behavioural, cognitive and social difficulties as a result of an acquired or traumatic brain injury. Simone’s team leader role involved the management of support workers and ensuring they performed their roles safely and to the best of their ability. Simone organised the monthly rota and managed annual leave requests and cover for the team.

Simone successfully balanced her Occupational Therapy master’s degree alongside her assistant case management role (ACM) with CCMS. She was able to apply occupational therapy theory, approaches and knowledge into real life scenarios. During her ACM role, Simone demonstrated excellent ability in setting up, supporting and managing a care team. She provided support and regular supervision to support workers, ensuring their wellbeing, whilst encouraging and striving for high quality patient care. Simone helped organise fun and meaningful activities, day trips and holidays to optimise clients functioning and quality of life. Simone regularly reflects on her practice and engages in CPD to maintain and develop her clinical knowledge and experience.

As a Case Manager, Simone optimally coordinates a clients care and rehabilitation programme with a goal focused and therapeutic approach. Simone sources and appoints appropriately trained, skilled, and experienced treating professionals that form a multi-disciplinary team. Multi-disciplinary teams are an effective tool to facilitate collaboration between professionals and hence improve outcomes for our clients. Simone supports the client, as well as their family, support workers and treating professionals to ensure the clients rehabilitation goals are successfully implemented and achieved. Simone will regularly review goals to ensure interventions are tailored towards their specific needs. Simone understands the importance of effective communication and organisation skills, and ensures that clients views, wishes and interests are central to all decision making.

Simone has followed the Community Case Management Services Ltd Pathway and has a sound understanding of the litigation process,the importance of record keeping and documentation, and managing a team of professionals whilst consistently following a holistic approach to rehabilitation.

PERSONAL PROFILE

Simone appreciates the importance of maintaining a good work-life balance and enjoys regular exercise and relaxation. She attends the gym and a running group four to five times a week and has recently completed a half marathon. Simone’s future goal is to run a marathon.
