Jennifer is an experienced Practitioner Case Manager supporting children, young people, and adults. Her work spans a wide range of complex needs, and she is passionate about providing person-centred, holistic support that empowers clients and their families to live fulfilling lives. Her professional background covers a variety of conditions including traumatic brain injuries, cerebral palsy, orthopaedic injuries, and amputations. Alongside physical health challenges, Jennifer has worked extensively with mental health and behavioural needs.
She has supported clients and families through complex safeguarding issues. In her role as a case manager, Jennifer has led and coordinated multidisciplinary treating therapists to ensure each client’s needs are fully addressed. She has been responsible for recruiting, organisation of mandatory and client specific training. Supervising care teams, ensuring that care packages are not only consistent and reliable but also compliant with regulatory standards. Her work is both practical and strategic. She has overseen housing adaptations, supported families through the EHCP application process, and coordinated reintegration into the community, schools and vocational placements.
A large part of her role involves guiding and supporting families —helping parents and siblings navigate the complexities of health, education, and social care, while also arranging respite and community support to strengthen family wellbeing.
Jennifer also has significant experience within the litigation process, preparing clear and detailed documentation for medico-legal purposes and working closely with deputies and solicitors to ensure costs are monitored and funds are managed responsibly. Jennifer has supported clients through life-changing transitions, including coordinating adapted housing and a dedicated MDT for a client with cerebral palsy, and guiding a young client with complex amputations through specialist rehabilitation to maximise independence.
Jennifer’s journey into case management began as a support worker, before moving into a team leader role and then assistant case manager. These early roles gave her a real insight into the everyday challenges faced by clients and their families, and shaped her passion for person-centred care. Growing up with a brother who has cerebral palsy, Jennifer has always understood the importance of ensuring that people with additional needs are supported to live full, independent lives. Now, as a twin mum herself, she brings empathy, resilience, and a strong sense of advocacy into her work.
Outside of work, Jennifer is passionate about running and cold-water swimming, which help her maintain balance and energy in both her personal and professional life. These pursuits reflect her belief in resilience, perseverance, and wellbeing — values that are also at the heart of her approach to case management.
