Immediate Needs Assessments are an intrinsic part of the case management process, but at CCM we have introduced an additional Care Needs Assessment that allows us to focus on the ongoing care needs of the client alongside a plan that helps us support them in achieving their goals.
The care needs assessment process captures information about their index injury, any diagnosed health/ mental health conditions, any diseases/ infections, any sensory/cognitive impairment and any medication and nutritional needs. This assessment identifies what care needs are being met informally or partially or not being met at all, how these needs could better be met with formal care provision in place and what of a number of options is recommended.
As part of this assessment, our CCM case managers are competent and skilled in setting out provision and costing to support accessing or utilising funding from a variety of sources. At CCM we make it our mission to find out what’s important to the client, what makes them happy and what their goals are. This allows us to develop a truly tailored care plan for the client, which is bespoke person centred, and reviewed as often as is need as goals are achieved and new ones identified! After all life is for living! A Care Needs Assessment can be offered as a standalone assessment and report, or it will form part of the case management process and plan whereby care is crucial.
At CCM we offer a fully regulated case management service across England & Wales, supported by a multi-disciplinary team of highly specialist treating professionals and therapists. To provide our clients with the very best of care and rehabilitation, we have an established due diligence process to ensure that all independent treating professionals sourced by CCM are of the highest calibre, equipt with the specialist knowledge and skills aligned with the specific needs of the client.
The therapy team sourced and appointed will ordinarily include Occupational Therapists, Physiotherapists, Neuropsychologists, Dieticians and Speech and Language Therapists, although this is not exhaustive.
As one of the longest running, and leading case management companies in the UK, CCM work with and support the wide network of interested parties that are involved in the client’s recovery.
Our team work closely with the client’s family, their Deputy and all of the other professional involved in the process including the Claimant and Defendant Solicitors, Insurers and any relevant Health Servies and Social Care Professionals providing statutory services.
Our Senior Management Team meets every week to complete a critical case review allowing all cases to be discussed and progressed accordingly as part of our overall clinical governance approach, to support best practice.
Our aim is to ensure that our clients feel comfortable and have a say in the decisions being made about their support programme. We want them to choose who supports them and who will be there throughout their journey.
It's essential that clients remain in control of the process in order to build a positive connection to the case manager and their support network.
