Jess Warburton is an experienced Case Manager providing services to children and adults across the Midlands and North-West. Her specialist experience includes paediatric neurorehabilitation, complex neurological presentations, and complex orthopaedic rehabilitation across both community and educational settings.
Jess brings a structured, outcome-focused approach to complex case coordination, with a strong emphasis on goal-based rehabilitation, measurable outcomes, and quality-of-life enhancement
Jess has extensive experience leading and coordinating multi-disciplinary teams, ensuring that all professionals work from a shared, client-centred goal framework. She regularly instructs and oversees MDT input to ensure interventions are purposeful, proportionate, and aligned with agreed rehabilitation objectives. Jess is experienced in managing and monitoring rehabilitation budgets, ensuring resources are used effectively and transparently to support meaningful outcomes
A core element of Jess’s practice is the identification of the client’s own goals, values, and aspirations, ensuring rehabilitation planning reflects what matters most to the individual and their family. Baseline assessment is undertaken using a biopsychosocial framework, allowing physical, psychological, social, and environmental factors to be considered holistically. This provides a clear structure for outcome measurement, review, and progression.
Jess places strong emphasis on therapeutic leisure, education, and vocational activities, recognising their central role in improving engagement, independence, emotional wellbeing, and overall quality of life. She actively supports clients to access meaningful and adaptive activities that reflect their interests, abilities, and stage of life, viewing participation as a core rehabilitation outcome rather than an adjunct to therapy.
Outside of her professional work, Jess enjoys a range of practical and active interests. She is a mum to two young boys, and family life plays an important role in her day-to-day routine. Much of her time outside work is spent supporting her sons’ sports and leisure activities and enjoying an energetic, outdoor lifestyle together.
Jess is a keen horse rider and enjoys training to compete, valuing the discipline and consistency this requires. She also has a strong interest in property renovation, particularly projects that involve careful planning, problem-solving, and long-term improvement.
