Sarah-Jane is a Registered Nurse and BABICM Registered Practitioner Case Manager with a professional background spanning public health, psychosocial research and complex community care coordination. Her practice is structured, analytical and firmly centred on each client’s individual goals and aspirations.
Her clinical experience includes supporting individuals living with dementia, neurological conditions and complex long-term needs within both home and residential settings. In addition, her work as a Membership Engagement Officer within the learning disability sector strengthened her advocacy skills and reinforced her commitment to inclusive decision- making and amplifying the client voice.
Sarah-Jane works collaboratively with clients and families to establish meaningful, realistic goals and ensures all professionals operate within a clearly defined, goal-based platform with measurable outcomes. She has experience coordinating multidisciplinary teams and provides clear instruction and oversight to ensure interventions remain purposeful, outcome-focused and regularly reviewed.
She undertakes comprehensive baseline assessments using a biopsychosocial framework, drawing on both her expertise and her academic foundation in Psychosocial Sciences. This enables her to consider not only physical health needs, but also cognition, emotional wellbeing, behaviour, social context and environmental influences when developing care plans.
She is confident managing rehabilitation programmes and associated budgets, ensuring resources are allocated effectively and transparently to maximise progress and value. Her approach combines clinical reasoning with practical organisation, promoting safe, sustainable and personcentred care delivery.
She recognises the importance of therapeutic leisure activities in improving confidence, independence and quality of life, and seeks to incorporate meaningful activity into rehabilitation planning wherever appropriate. Her earlier career in public health intelligence and programme development continues to inform her structured, evidence-informed and systems-aware approach to case management.
Outside of her professional role, Sarah-Jane is a beginner archer and values the focus and patience the sport requires. She also enjoys spending time with her large extended family. She values balance, resilience and purposeful growth, both personally and professionally.
