A brief introduction to some of our Associate Case Managers who have capacity to take on clients.

 

Lena Walliman

Lena’s approach to CM work “is incredibly personalised and bespoke and no size fits all…    It’s all about personality, approach and ability to read the room”

Lena is a highly skilled Specialist Nurse Practitioner and fantastic expert witness. Lena has independent prescribing qualifications and extensive experience managing diverse client healthcare needs across public and commercial sectors. She adopts a transparent, responsive approach, building team trust while expertly identifying organisational issues, restructuring teams, and clarifying roles.

Lena inspires individuals to focus, take ownership, and achieve results, while forging stakeholder partnerships to enhance services. With expertise in risk management, governance, and SIRO responsibilities-as a former board member of a national specialist clinical group-she ensures excellence in governance and board performance. Lena has published peer-reviewed guidelines and articles in nursing journals, served as a CQC registered manager, and provided national oversight of care provision across England, Wales, and Scotland.

Anna Maria Haritakis 

“ I believe it is valuable to listen and build a trusting relationship with the person and family I am working with, in order to support access to the resources they need.  I take a collaborative, holistic, compassionate and flexible approach to each person’s goals and situation.”

 Anna-Maria graduated in 2014 from York St John University with a BSc (Hons) in Physiotherapy. She has extensive community-based experience treating adults and paediatrics, including rehabilitation units, and has been an associate case manager with CCM since 2022.Adopting a holistic approach, she specialises in complex health needs and life-changing injuries, working closely with clients and families. With in-depth knowledge of community services, she ensures optimal access to support rehabilitation. Anna-Maria excels at coordinating multidisciplinary teams for collaborative, goal-oriented care, identifying additional supports, and achieving best outcomes.

 Experienced in catastrophic cases, she partners with OTs, GPs, health professionals, and voluntary sectors, advocating for families while leveraging strong community networks. Trained in Health Coaching, she educates and empowers clients and families, managing expectations and promoting active rehabilitation participation.

 Paul Owen

 Paul is an experienced Brain Injury Case Manager, he has developed extensive knowledge of community services and placements for those with acquired brain injury. Gained experience in benefits, housing, homelessness, immigration status, safeguarding concerns, Best Interests Meetings, family disputes, Mental Health Tribunals. Engaging with commissioners and agencies such as NHS England.

Paul is a registered nurse with over 30 year’s experience in the health and social care sector. He has a wealth of experience in working with individuals with a variety of complex and challenging needs. Paul is able to quickly develop rapport with individuals and maintain a calm approach in difficult situations. He is skilled in assessing client’s needs and identifying recommendations and from these liaising and leading MDT’s as required. He has vast experience managing care teams and is able to lead these effectively.

 Jennifer Duignam 'Experienced Case Manager with 5+ years’ experience and a specialist SEN background. I combine professional expertise with lived family experience of cerebral palsy, taking a compassionate, collaborative and holistic approach. Level 3 BSL, committed to clear communication and meaningful outcomes.

 Jennifer is an experienced Practitioner Case Manager supporting children, young people, and adults. Her work spans a wide range of complex needs, and she is passionate about providing person-centred, holistic support that empowers clients and their families to live fulfilling lives. Her professional background covers a variety of conditions including traumatic brain injuries, cerebral palsy, orthopaedic injuries, and amputations. Alongside physical health challenges, Jennifer has worked extensively with mental health and behavioural needs.

 Jennifer’s journey into case management began as a support worker, before moving into a team leader role and then assistant case manager. These early roles gave her a real insight into the everyday challenges faced by clients and their families, and shaped her passion for person-centred care. Growing up with a brother who has cerebral palsy, Jennifer has always understood the importance of ensuring that people with additional needs are supported to live full, independent lives. Now, as a twin mum herself, she brings empathy, resilience, and a strong sense of advocacy into her work.

 Richard Corner  ‘I feel my approach to case management is rooted in person centred, holistic care and bringing together multidisciplinary teams to provide an outstanding service for the person and family I’m supporting.

 As a Clinical Lead working for a national care company, Richard has been instrumental in driving quality improvement initiatives, ensuring best practice, and consistently delivering high-quality care outcomes. He possesses significant experience in leading and coordinating multidisciplinary teams (MDTs), fostering collaborative environments, and ensuring all stakeholders work cohesively towards patient goals. He excels at developing and implementing individualised, goal-based care plans with measurable outcomes, effectively managing associated risks and resources to ensure optimal client benefit.

 His experience as an Expert Witness further highlights Richard’s ability to interpret complex medical information, particularly in the context of medico- legal cases. He has collaborated with solicitors, providing expert nursing opinions and detailed reports that clearly explain complex medical circumstances and their implications, ensuring that legal professionals have the precise and accessible information needed for their cases.

 Olga Chernyavska

 Olga has extensive experience creating person-centred support plans that integrate therapy goals with clients' aspirations to improve biopsychosocial functioning and quality of life. She conducts regular risk- and need-led reviews of care plans and rehabilitation programmes, having served as a reviewing officer for vulnerable adults in community, extra care, and care home settings since starting her career in Adult Social Services in 2008.

 Trained in minor adaptations and equipment, she assesses and procures items to enhance daily living. With an MA in Social Work (2016), Olga excels in Care Act 2014 assessments, remote/face-to-face reviews, CHC checklists, DST multi-agency meetings for NHS funding, Mental Capacity Assessments (per MCA 2005), and carer assessments. Her strengths-based approach promotes preventative measures and independence.

 In the Post Enablement Team, she developed pathways and best practices for optimal outcomes, while in safeguarding, she conducted investigations and risk management plans. Olga has consistently mentored new staff and students, offering shadowing, role clarification, and support for high-quality assessments and reviews.
